Death investigation report still incomplete

Four months after a worker’s death in Seneca, the report of an investigation of his death remains incomplete, according to a spokeswoman for the applicable state agency. Lesia Kudelka, ombudsman and communications director for the state Department of Labor and Licensing Regulation, said she checked yesterday for the status of the death investigation report from the Occupational Health and Safety Administration and learned that the report is still not ready. The OSHA office investigated the November accident that took the life of Liberty resident Charles Keeler, who died while working as a contractor’s employee at the Seneca Water Treatment Plant.
